HOSPITAL BILL

Your hospital bill will include your room charge, medications, tests, supplies, etc. If you have questions regarding your bill please call the Business Office at (660) 269-3142 between 8:00 a.m. and 5:00 p.m. M-F. Please have your patient number and date of your visit available. You may also write

Your hospital bill does not include fees for the professional services of your physician, surgeon, anesthesiologist, radiologist, pathologist and consulting physicians. You will receive separate billings for services provided by these specialists.

MEDICAL RECORD INFORMATION

If you require a copy of your medical records, please call (660) 269-3134. By obtaining the answers to a few questions in advance, our Health Information Services Department will be able to have your records ready for you when you come to the hospital to pick them up. You will need to complete an Authorization for Release of Medical Records. This consent form must be signed and dated by the patient or legal representative. The first copy is free with a small fee applied for additional copies. Please give 24-48 hour notice prior to coming in and picking up your records.
